Aging with Dignity is a non-profit agency that provides practical information, advice and legal tools including “Five Wishes”.  Five Wishes is a document that can be completed by an individual to let their friends and family know their wishes for end of life care.

The Well Spouse Association is the only national organization focusing exclusively on the needs of spouses caring for a chronically ill and/or disabled husband, wife or partner.
