RN Case Manager	Primary Responsibilities: Administer skilled nursing care for clients in their place of residence. Case manage and provide clinical directions to the interdisciplinary team, patient/ family and referring agency; and assume the responsibility for coordination of care. Assure continuity of quality patient care is delivered with appropriate documentation. Demonstrate commitment, professional growth and competency. Requirements: Must be a graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing. Must hold a current Colorado State license as a Registered Nurse. Clinical Supervisor	Primary Responsibilities The Clinical Supervisor is to coordinate and manage all activities, assume responsibilty for continuity, appropriateness and quality of services delivered by the agency. Primary Functions Supervise and provide direction and leaderhip to clinical personnel. Competently perform patient care assignments and staff management activities. Assess, plan, implement and supervise patient care. Participate in the agency QA/PI activities. Monitor cases to ensure compliance with regulatory agencies. Monitor assigned cases to ensure compliance with third party payors. Promote agency philosophy and mission and administrative policies to ensure quality care. Requirements Must be a graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing. Must hold a current Colorado State license as a Registered Nurse. Must have at least two years experience as a registered nurse and one year in home care (management experience highly preferred)! Must hold a current Colorado State Driver License. Good interpersonal skills, proof of current CPR and Hepatitis Profile, reliable transportation with current, valid auto liability insurance.
