RN Case Manager  Primary Responsibilities: *Administer skilled nursing care for clients in their place of residence. *Case manage and provide clinical directions to the interdisciplinary team, patient/ family and referring agency; and assume the responsibility for coordination of care. *Assure continuity of quality patient care is delivered with appropriate documentation. *Demonstrate commitment, professional growth and competency. Marketing Representative  Primary Responsibilities: The Marketing Representative will serve as a liaison between The Agency & the community regarding community and customer needs. *Development and promotion of home health programs and services while maintaining the standards of practice consistent with quality health care and maximizing human, financial and equipment resources. Qualifications: *Must be a graduate of an accredited college or university. *Previous experience in a sales capacity within the health care arena is required; home health or hospice preferred. *Must have the ability to establish and maintain effective working relationships with all segments of the staff, and community as well as professional contacts, the Board of Directors, Advisory Board and Medical Director. *Must have strong sales and customer service skills. *Must have the ability to work under a variety of conditions in facilities and offices; the ability to travel locally; and ability to work a flexible schedule.
