RN Case Manager	The RN’s primary functions are to administer skilled nursing care for clients in their place of residence, coordinate care with the interdisciplinary team, patient/ family and referring agency; and assume the responsibility for coordination of care. Must be a graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing, current Iowa State license as a Registered Nurse, current Iowa State Driver License, minimum of 6 months experience as a Registered Nurse in a clinical care setting, home health preferred, good interpersonal skills, proof of current CPR and Hepatitis Profile, reliable transportation with current, valid auto liability insurance.
