RN Case Manager	Primary Responsibilities: Administer skilled nursing care for clients in their place of residence. Case manage and provide clinical directions to the interdisciplinary team, patient/ family and referring agency; and assume the responsibility for coordination of care. Assure continuity of quality patient care is delivered with appropriate documentation. Demonstrate commitment, professional growth and competency. Requirements: Must be a graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing. Must hold a current Florida State license as a Registered Nurse. Must hold a current Florida State Driver License. A minimum of 6 months experience as a Registered Nurse in a clinical care setting, home health preferred. Good interpersonal skills, proof of current CPR and Hepatitis Profile, reliable transportation with current, valid auto liability insurance. We Offer: Low Nurse/Patient ratio! PRN LPN	Primary Responsibilities: Administer skilled nursing care, under the supervision of a Registered Nurse, for clients in their place of residence. Coordinate care with the interdisciplinary team, patient/ family and referring agency. Assure the continuity of care through delivery of quality patient care. Monitor assigned cases to ensure compliance with requirements of third party payer. Requirements: Must be a Graduate of an accredited school of Vocational Nursing. Must hold a current Florida State License as a Licensed Vocational Nurse. Must have a current Florida Driver License. A minimum 6 months experience as a Licensed Vocational Nurse in a clinical care setting, home health preferred.
