LPN	We are currently accepting applications for a full time LPN for our East Valley service area. Primary Responsibilities: Administer skilled nursing care, under the supervision of a Registered Nurse, for clients in their place of residence. Coordinate care with the interdisciplinary team, patient/ family and referring agency. Assure the continuity of care through delivery of quality patient care. Monitor assigned cases to ensure compliance with requirements of third party payer. Requirements: Must be a Graduate of an accredited school of Vocational Nursing. Must hold a current Arizona State License as a Licensed Vocational Nurse. Must have a current Arizona Driver License. A minimum 6 months experience as a Licensed Vocational Nurse in a clinical care setting, home health preferred.
